Editing and Creating Questionnaires in NorthGRC for Vendor assessments
In this guide, we will go through how to edit the questionnaires that can be sent out for vendor assessments. Furthermore, the guide will explain how you can create your own questionnaires from scratch.
Editing Existing Questionnaires
To edit the existing questionnaires, follow these steps:
- Navigate to Settings → Content → Templates.
- Perform a quick search using the keyword 'Vendor questionnaire'.
- This search will display three questionnaires. These are the default questionnaires provided by the system.

If you open the questionnaire titled 'Vendor Questionnaire: Information Security,' you will access the editing mode for the questionnaire. Here, you will see the elements that make up the questionnaire. The questionnaire consists of a mix of chapter headings, dropdown menus, and formatted text fields. Here are some examples of how the different fields look.

The fields can be freely edited and customized to suit your needs. If the existing text and response options align with what you want to send to your suppliers, you can also choose to keep the questionnaire as it is.
You have the option to add new fields to the template by hovering between existing fields, where a small blue plus sign will appear. Simply click on it to add a new field:

Here you have the option to add additional fields, which are divided into the following categories: Add Content, Add Field, Add Risk Field, and Add ESG Field.
The Add Field category contains the fields typically used in questionnaires, as they can be edited in view mode:

Since vendors are typically only required to fill in the fields with information—either as free text or by selecting from predefined options—these fields make the most sense.
Creating new questionnaires
Under 'Templates', you have the option to create your own templates by clicking on '+ Create template'.

The new template will open in a new window.

Here, as in the already created questionnaires, you can freely add new fields by clicking on the blue plus icon.
If you want to add dropdowns that have not already been created in other surveys, you need to navigate to 'Data Lists' under Content and create a new data list with the options that the dropdown menu should contain. Afterward, when you add a dropdown field to the template, you will have the option to select the data list you just created. You do this by adding a new field and choosing 'Dropdown':

When dropdown is selected, you need to fill in a heading, a description of the dropdown menu (this is optional), and finally select the dropdown you have just created under Data Lists.

If you would like to base you questionnaire on an existing questionnaire, you can copy an existing questionnaire by clicking the copy icon on the right side of the Template bar, where you will then need to select which language the copy should be copied in.

Afterwards, the system will create a copy of the selected questionnaire, which will appear alongside the existing questionnaire, but with green text.

You are free to edit the copy without affecting the original template, and you have the same options as if you were creating a questionnaire from scratch.
You can find a guide on sending vendor questionnaires.
